Visa Requirements

1. Passport Copy (clear scanned copy) 
2. Filled out form 
3. Family Info Sheet ( pls complete) 
4. COE / ITR / Cert of Leave – if employed 
5. DTI or SEC/Business Permit/ITR – if business owner 
6. Original Bank Cert 
7. Certified True Copy of Bank Statement ( 6months ) 
8. Travel History ( copies of the entry / exit / visa ) 
9. ID Picture ( 4.5cm x 3.5cm ) 

Other proof of financial capabilities:  

Credit Card bank statement with payment  

Title copies for properties owned  

Investments/Shares of Stocks certificate  

OR/CR of owned vehicle  

if sponsored, ALL FINANCIAL PROOF must be provided by the  

sponsor, visa copy, employment contract or Certificate of employment,  T1/T4 copy, invitation letter indicating the address, email address, contact #, passport copy of the sponsor, and proof of relationship like a birth certificate. All docs must be notarised in Canada.  

Tourist Visa/Biometrics/Professional Fee: PHP10800  

Super Visa/Biometrics/Professional Fee: PHP14000 

****Medical may be requested for a Super visa and the medical fee is payable to the accredited clinic.
