3. COE / ITR / Cert of Leave – if employed 
4. DTI or SEC/Business Permit/ITR – if the business owner 
5. Original Bank Cert 
6. Certified True Copy of Bank Statement (6 months) 
7. Travel History ( copies of the entry/exit / visa ) 
8. ID Picture ( 4.5cm x 3.5cm ) 

Other proof of financial capabilities:  

Credit Card bank statement with payment  

Title copies for properties owned  

Investments/Shares of Stocks certificate  

OR/CR of owned vehicle  

if sponsored, all financial proof must be provided by the sponsor,  

visa copy, employment contract if any, invitation letter, passport  

copy of the sponsor and proof of relationship like a birth certificate.
